Richard Henderson e1d8fabc20 tcg: Define INSN_START_WORDS as constant 3 
Use the same value for all targets.

Rename TARGET_INSN_START_WORDS and do not depend on
TARGET_INSN_START_EXTRA_WORDS.
Remove TCGContext.insn_start_words.
