scsi: move request lists to QTAILQ.

Changes:
 * Move from open-coded lists to QTAILQ macros.
 * Move the struct elements to the common data structures
   (SCSIDevice + SCSIRequest).
 * Drop free request pools.
 * Fix request cleanup in the destroy callback.
